Free Whitening for Life Program
OFFER TERMS, FEES, CONDITIONS & RESTRICTIONS
To qualify for Full Smile Dental’s Free Whitening for Life Program, all individuals must:
- Be 18 years of age or older
- Disclose to Full Smile Dental all medical conditions, medications, supplements, and allergies
- Have a healthy mouth free of dental disease
- Be cleared by a Full Smile Dental professional to participate in the Program
- Receive a comprehensive dental examination and full mouth X-rays at Full Smile Dental within 24 hours before starting the Program (additional terms and fees may apply)
- Keep twice yearly hygiene/cleaning appointments at Full Smile Dental in order to continue receiving complimentary bleach syringes
- Pay a $150 enrollment fee
The Full Smile Free Teeth Whitening for Life Program requires an enrollment fee of $150. Then, the program includes a take home bleaching kit, and up to two (2) additional complimentary bleach syringes per year. Patients must keep their preventive visits current (to include a minimum of twice yearly cleaning appointments) and provide at least a 24-hour cancellation notice prior to any Full Smile Dental appointment in order to receive a complimentary bleach syringe. One (1) complimentary bleach syringe will be dispensed in 6-month intervals at time of hygiene/ cleaning appointment.

Side effects of teeth whitening may include, but are not limited to:
- Tooth Sensitivity
- Gum Irritation
- Teeth Staining
- Allergic Reactions to Bleaching Agents and Materials
